어메이징 슬롯·퇴학·유학 등 / Leave of Absence / Withdrawal / Study Abroad.

어메이징 슬롯, 복학, 퇴학, 유학, 전학부·전학과, 구성 사용 등을 희망하는 경우는 대학에 소원하여 허가를 받아야 합니다. 수속에 있어서 주의해야 하는 경우도 있으므로, 가능한 한 빨리 소속학부(연구과) 교무 담당 창구에 수속 방법 등의 상세를 문의해 주세요.

또한, 월요일을 거슬러 올라가 소원할 수 없습니다.
　
University permission is required by student application for the following: - leave of absence, return to school, withdrawal, study abroad, transfer of faculty, transfer of department, change of maiden name. The procedure may be complicated, so please contact the your faculty as soon as possible to enquire about details of procedure, method etc.

It is not possible to change dates for these applications once they have been initiated.

성명, 주소 등 변경 / Change of Name, Address.

본인이나 가정 연락처의 주소·연락처(전화 번호나 E-mail 등)에 변경이 있었을 경우는, 신속하게 학무 시스템에
변경 내용을 입력하세요.

수업료 납부서 송부처나 보증인의 변경이 있는 경우는, 소속 어메이징 슬롯(연구과) 회계 담당 창구에 신고해 주세요.

If there is a change in any of your relevant addresses, contact information (phone numbers,
e-mails etc) or your home contact address, please promptly enter the changed details into
the academic system.

If there is a change in the billing details for your tuition fee payments or your guarantor,
please notify it to your departmental or graduate school accounting office.

개성명·본적 변경을 했을 경우는, 소정의 신고 용지에 기입한 후, 소속 어메이징 슬롯(연구과)의 교무 담당 창구에
제출해 주세요.

덧붙여 개성명의 신고를 했을 경우, 성명을 변경한 새로운 학생증을 발행해, 학무부 학무 기획과로부터 연락
　합니다. (신청 필요 없음)

If you have changed your name and/or domicile, fill in the prescribed form and submit it to
The Academic Affairs and Student Office of your faculty or graduate school.

In addition, if you have notified us of the changed name, we will issue a new student card
　with the new name, and will contact you from the Academic Affairs Department, Planning Division
(Application for a new student ID Card is unnecessary).

구성·통칭 사용을 희망하는 경우는, 소속 어메이징 슬롯(연구과)의 교무 담당 창구에 상담해 주세요. 덧붙여 통칭이란 호적상의 성명(이하 「본명」) 대신에 본명 이외의 호칭으로 본명을 대신하는 것으로 널리 통용하고 있는 것(구 성을 제외한다.)입니다.

If you wish to use your maiden name or a common name, please contact the Academic Affairs and Student Office, which is your faculty or graduate school.
A common name is a name other than the real name that is widely used in place of the name on the family register(excluding maiden name).

이름의 영문자 표기

본학에서는, 성명의 영문자 표기를 2014년 11월 재학생보다 어메이징 슬롯 통일하고 있습니다.

일본 국적의 경우:
원칙적으로 헤본식 로마자 방식에 따릅니다. 단, 여권의 기재가 헤본식에 의하지 않는 경우는
여권 사본을 지참하신 후 소속 어메이징 슬롯(연구과) 교무 학생 담당자에게 신고하십시오. (소정 양식을
기입하고 여권 사본과 함께 제출합니다. )
또한, 성명순, 대문자 소문자의 기재는 다음과 같이 합니다.
　　예)오카다이 모모코(성)OKADAI(이름)Momoko

외국적의 경우:
원칙적으로 본인의 제안대로 합니다. 다만, 인쇄할 수 있는 문자수에 제한이 있기 때문에, 개별적으로 상담시켜
받을 수 있습니다.
