When you arrive before 8:30AM, please take a number at the 1st floor information counter.
Closed/  Saturdays, Sundays, national holidays, and the year-end and New Year holidays

• Please make sure to bring a referral letter, My Number Card, Health Insurance Eligibility Certificate, and Recipient Certificate.

• A patient without a referral letter must pay "Elective Treatment Fee" of 5,500 yen(inc.tax) in addition to the consultation fee.

Fill out the Registration Form, and show a referral letter, My Number Card or Health Insurance Eligibility Certificate, and Recipient Certificate at Counter No. 1.
 

After registration, you will receive a patient ID card and an Registration Slip.
